Essential Duties and Responsibilities: (other duties may be assigned):
- Assist residents with ADLs as indicated in individual service plans.
- Assist with meal service.
- Assist with laundry.
- Provide 1:1 resident escort as necessary for outings and appointments.
- Respond to emergency situations; administer First Aid as needed.
- Observe and report to nurses any changes in residents’ physical, cognitive, or emotional condition.
- Obtain and document vital signs as assigned.
- Handle all forms of resident Protected Health Information (PHI) in a confidential, private, and secure manner.
Equipment Used: Stethoscope, Blood pressure cuff, transfer belt, computer kiosk.
Supervisory Responsibility: None
QUALIFICATIONS
To perform this position successfully, an individual must be able to perform each essential duty satisfactorily. - Ability to work independently and in a team.

- Ability to function as a team member in sharing responsibility for the provision of resident services in cooperation with all other members of the team.
- Must be able to communicate clearly both verbally and in writing.
- Must be able to read/write/speak English.
- Ability to speak Japanese a plus.
- Must enjoy working with the elderly and demonstrate an understanding and appreciation of the aging process.
Education, Experience and Certifications:
- Current Washington State Nursing Assistant Certification or Home Care Aid Certification.
- Current CPR and First Aid certification or must obtain within 30 days of hire.
- Dementia Specialty Training and Mental Health Specialty Training or must obtain within 120 days of hire.
- Current Food Handlers permit or must obtain within 14 days of hire.
- HIV/AIDS training or must obtain within 30 days of hire.
- Must submit to a Name & Date of Birth state background check upon hire and every 2 years thereafter.
- Fingerprint Checks: Fingerprint checks must be conducted one time only upon hire for all administrators, nurses, and caregivers.
Physical Requirements:
- Standing/Walking: while completing job responsibilities.
- Sitting:while completing job responsibilities.
- Lift/Carry: work related materials, reports, mail, etc.; up to 50 lbs.
- Push/Pull: while completing job responsibilities.
- Climbing: while completing job responsibilities.
- Bending/Twisting: while operating office equipment, filing, accessing filing cabinets.
- Hands/Arms: use of both hands and arms in reaching, handling, grasping, fingering, gripping throughout the work shift.
- Sight: use with visual acuity and near/mid/far range of vision, depth perception and peripheral vision required.
- Speech/Hearing: used performing essential job functions and communicating with co-workers, residents, visitors and supervisor.
Constantly = over 2/3 time; Frequently = 1/3 – 2/3 time; Occasionally = under 1/3 time
Mental Requirements:
- Must have good memory, recall and be detail oriented.
- Must be able to use sound judgment (problem resolution skills) in the execution of daily tasks.
- Must be able to carry out verbal and/or written instructions.
- Must be able to effectively present information and respond to questions from groups of managers, nurses, residents, family members and co-workers.
- Must be able to communicate clearly both verbally and in writing.
- Must have excellent interpersonal skills and be able to work effectively in a team environment.
- Must be able to read/write/speak English.
- Must possess basic mathematical skills.
- Fluency in Japanese language is a plus.
