What You’ll Do:

– Conduct assessments of patients and families to identify home care needs

– Perform physical evaluations and document medical history

– Develop, initiate, and adapt care plans with family involvement

– Prepare discharge plans in advance

– Educate and support patients and families on ongoing care needs

Requirements:

– RN license from an accredited nursing program

– Minimum 1 year of recent RN/LVN/LPN experience, preferably in home health, public health, or hospice

– Active state licensure and CPR/BLS certification

– Valid driver’s license, insured vehicle

– Strong communication, problem-solving, and clinical skills
