TeamHealth is hiring a dedicated and compassionate licensed professional counselor, licensed mental health counselor, or licensed marriage and family therapist to join our behavioral health team serving three skilled nursing and long-term care facilities in the Rockford, Illinois, area.

Your Role

• Provide psychotherapy and behavioral health services to residents in skilled nursing and long-term care settings
• Conduct assessments, develop individualized treatment plans, and deliver ongoing therapy
• Collaborate with interdisciplinary care teams to promote emotional and psychological well-being

What We’re Looking For

• Active LPC, LMHC, or LMFT license in Illinois (all supervisory hours completed)
• Eligible for independent practice and billing
• Strong clinical, diagnostic, and communication skills
• A genuine commitment to improving the mental health of older adults
