WHO WE ARE

Amae Health is a Series B, venture-backed Public Benefit Corporation that delivers specialty outpatient care for people with severe mental illness. We build and operate high-acuity, in-person clinics nationally.

Our model integrates psychiatry, therapy, primary care, dietitians, and community support under one roof. But what makes us fundamentally different is that we are also building the science and technology layer that this field has never had. We are developing proprietary clinical protocols, designing novel measurement science, and engineering an AI data science platform purpose-built for the complexity of severe mental illness. Amae Health is seeking an energetic and resourceful Community Liaison to join our Community Engagement Team. This full-time role requires a home address within the New York metro region.

The Opportunity:

We’re actively expanding in the New York City market and seeking a dedicated Community Liaison to support this growth. In this role, you’ll build and maintain strong relationships with community stakeholders, advocate for our innovative care model, and champion Amae’s mission to deliver compassionate, comprehensive mental health services to individuals with serious mental illness.

You’ll be deeply embedded in the community, conducting outreach to local and regional partners, including healthcare providers, hospitals, community agencies, correctional facilities, treatment centers, and other referral sources, to promote Amae’s integrated care model and develop sustainable partnerships.

As an ambassador for Amae Health, you’ll thrive in a dynamic environment, balancing tasks like engaging hospital systems and providers, coordinating complex care transitions, responding to urgent referrals, visiting emergency departments, and tracking key details to ensure seamless patient experiences. Your success will come from blending strategic relationship-building with hands-on coordination and problem-solving to strengthen Amae’s community presence.

What You Will Work On:

Community Outreach & Referral Partner Development – 80%

  • Actively manage and grow patient referrals in your region through systematic outreach to existing provider partners
  • Identify and develop new potential referral sources to grow Amae’s presence and ensure a steady pipeline of patient referrals
  • Coordinate rapid responses to urgent patient situations, collaborating with hospital systems, social workers, and providers to ensure timely care transitions.
  • Work closely with clinical staff to ensure that referred patients are connected with the appropriate care and support
  • Serve as a visible, trusted ambassador for Amae Health in your community

Planning, Collaboration, & Documentation – 20%

  • Track outreach activity, referral outcomes, and pipeline in CRM to meet or exceed enrollment targets
  • Develop quarterly market plans in collaboration with clinical and leadership teams
  • Participate in weekly team meetings with enrollment leads and market-based staff
  • Complete documentation accurately and on time

About You:

  • You’ve built relationships professionally and know how to open doors, follow through, and keep them open over time
  • You care about the people this work serves, and that shows up in how you communicate and problem-solve
  • You take ownership of your market without needing constant direction, but you know when to pull in your team
  • You stay organized in the field, keep your documentation current, and track what’s working so you can do more of it
  • You’re comfortable in fast-moving, emotionally complex situations and can shift easily between a hospital hallway and a family consultation
  • You’re coming from community health outreach, behavioral health sales, or a patient-facing role in a hospital or treatment setting, and you’re ready to own a market

What You’ll have:

  • 3+ years of client or customer-facing experience (e.g. in-person consulting, cold calling, digital outreach, business development, etc.), with defined engagement targets.
  • Proven track record of building a referral base and relationships, such as business development, sales, community outreach, or referral management
  • Knowledge of, and passion for, mental health and wellness
  • Excellent oral and written communication skills, including strong public speaking ability
  • Reliable transportation and ability to commute throughout the five boroughs via public transit, rideshare, or personal vehicle
  • General computer proficiency and ability to independently learn tools like Google Suite, Slack, and CRM systems

Compensation & Benefits

  • We pay competitively. Base salary reflects role, level, and geography. Total compensation includes valuable equity, comprehensive medical/dental/vision, unlimited PTO, parental leave, and programs built around employee well-being. We’ll share specifics during the interview process.

Base salary for this full-time role ranges from $75,000 to $105,000 per year. In addition to base compensation, this role is eligible for incentive compensation. Actual compensation packages are based on several factors that are unique to each candidate. These factors include, but are not limited to, job-related knowledge and skill set, depth of experience, certifications, degrees, licensures, and specific work location.
