Position Summary
The Hamilton-Madison House Caregiver Support Program provides essential services to informal and unpaid caregivers of older adults, as well as their loved ones, throughout all five boroughs of New York City. This citywide initiative prioritizes support for Asian-speaking communities, including Chinese, Korean, and Japanese speakers.
The Care Specialist plays a vital role in delivering these services by conducting telephone intakes and in-person assessments, screening clients for eligibility, and assisting them in applying for benefits and entitlements. Additionally, the Care Specialist provides information and referral services, while collaborating with MSW staff to develop and implement comprehensive care plans. The program also offers caregiver training, counseling, and support groups to ensure holistic support for caregivers and their families.
Responsibilities:
- Manage a caseload of both ongoing and short-term clients, ensuring effective and individualized support.
- Conduct telephone intake interviews to assess initial client needs and eligibility.
- Perform home visits using standardized assessment tools to thoroughly evaluate client requirements.
- Utilize screening tools to determine client eligibility for benefits and entitlements, assisting them with form completion, submission, and follow-up to ensure successful receipt of benefits.
- Maintain detailed and accurate case records in the database, including case notes and documentation of provided services.
- Collaborate on the development and delivery of caregiver training workshops designed to equip caregivers with essential skills and promote their own health and well-being.
- Actively promote the organization and its caregiver programs through outreach activities, participation in community events, and representation within the community.
Minimum Qualifications:
- Possess a Master’s degree in Social Work, Counseling, Psychology, or a related field.
- Bilingual proficiency in English and Chinese.
- Demonstrated experience in case management, social services, or a similar role.
- Strong communication skills, including expertise in telephone communication and the ability to build and maintain interpersonal relationships effectively.
- Comprehensive knowledge of benefits and entitlements, along with proven experience in completing and processing application forms.
- Capability to travel via public transportation and navigate stairs for conducting home visits and hosting information sessions.
- Proficient in basic computer skills, including familiarity with Microsoft Windows and Microsoft Office.
- Ability to work independently while also excelling in interdisciplinary team environments.
Hours: Monday to Friday, 9:00AM to 5:00PM, 35 hours per week
Salary Range: $48,000 to $52,000 depending on qualifications and experience
